OUR SERVICES

SKILLED NURSING

Provides care management, assessment and treatment of the patient's medical condition. The practice of home health nursing is focused on the care of the patient through collaboration with the physician, the patient, their family, and designated caregivers. Because there is often multiple disciplines involved in the plan of care, the home health nurse assumes the responsibility of coordinating all involved specialties and directing caregivers in order to optimize patient outcomes.

PHYSICAL THERAPY

Helps improve safety and independence in functional mobility (i.e. transfers, bed mobility, gait/ambulation).

OCCUPATIONAL THERAPY

Are skilled in evaluating a patient's functional abilities and environmental needs. They help restore patient's independence in activities of daily living (i.e. eating, dressing, grooming, bathing, etc.).

SPEECH THERAPY

Assists patients in improving their verbal and non-verbal communication skills as well as swallowing disorders. The speech therapist will assess the patient's speech patterns and their ability to understand spoken language. They will help the patient enhance their written, verbal, and non-verbal language skills in order to improve the patient's quality of life.

MEDICAL SOCIAL SERVICES

Helps to resolve social, emotional, and financial problems that may hinder treatment of stated rehabilitation potential. The MSW is also able to coordinate care and assistance from available resources.

CERTIFIED HOME HEALTH AIDE

Assists the patient with personal care and various daily living activities.

REGISTERED DIETICIAN

Provide education and assistance to patient in regards to their dietary plans best suited for their condition.

SPECIALTY PROGRAMS

Pre & Post Surgery Joint Replacement Therapy

Through intensive physical therapy prior to any joint replacement surgery, muscles are strengthened and pain levels are decreased, therefore reducing the need for in-patient rehabilitation after surgery. Our dedicated therapist are then able to treat the same post-surgery, providing continuity in care.

Cardio-Pulmonary (COPD)

This program is designed to help patients suffering from Cardio-Pulmonary Disorders such as Chronic Emphysema and Chronic Bronchitis. Patients are educated on healthy lifestyle habits that will reduce the risk of further complications or exacerbation, therefore reducing re-hospitalization and emergency room visits.

Cardiac Specialty

This program includes, but is not limited to the treatment of Coronary Artery Disease, Peripheral Vascular Disease, Post-Cardio Thoracic Surgery, and patients suffering from CHF, Angina Pectoris, Arhythmias, HTN, Arteriosclerosis, history of Myocardial Infarction, and Atrial Fibrillation. Health Source Home Care, Inc. utilizes the ZOE Fluid Status Monitor for qualified CHF patients, therefore recognizing symptoms of disease exacerbation up to two weeks earlier than normal weight gain.
